There are now three options for attending AEMTA EMS Conference 2008.

• Full Registration (4 days, conference bag, T-shirt and banquet ticket)
• 2-Day Registration (conference bag & T-shirt only)
• 1-Day Registration (Conference bag only)

When you complete your form, be sure to indicate which option you are selecting, and if you choose the 1 or 2-day option, please indicate which days you will be attending.

As always, please remember the following tips to make sure your registration is processed on time.

1. Please complete all personal information in its entirety. This information will only be used by the AEMTA and its societies and chapters.
   
   
2. Please indicate your T-Shirt Size. All 2-day and full conference attendees will receive a conference T-Shirt.
   
   
3. Attending the Banquet? Make sure you check whether or not you will be attending the banquet so that we may allow for enough seating.
   
   
4. Conference Registration – Please fill in the appropriate amount in the total column for your classification. One registration form per attendee. If you are not currently a member of the AEMTA then you may join the association and receive the discounted price for this conference.
   
   
5. AEMTA 2009 Membership Dues — You may pay your 2009 membership dues through this registration form. If you are joining the AEMTA for the first time through this registration form then you will be a member until December 31, 2009.
   
   Out of State EMS professionals, RNs, LPNs, Physicians, and non-EMS professionals may join the AEMTA as an Associate Member and receive registration discounts.
   
   Squad Memberships will not be accepted through the online process. Squad Memberships will be accepted through the printed conference registration form. The top portion of the registration form must be completed for each individual in a squad membership whether they are attending conference or not. Spreadsheets, e-mails, and letters listing individuals in a squad membership will not be accepted due to the information needed to keep our membership database accurate. One individual’s registration form must show the $175.00 fee and the other 19 individuals included in the primary membership need to be numbered 2-20 in the Squad Membership blank. For each additional membership beyond the first 20 the form will need to add $8.00 in the Total column blank.
   
   Societies may be joined for the 2009 membership year by writing the appropriate amount in the Total Column blank beside each society you want to join.
   
   
6. Total Due to Conference — Transfer the totals from the previous three columns to the appropriate corresponding blanks in this section and tally the grand total for the TOTAL DUE.  If you desire you may contribute to the Wannell Bradshaw Benevolence Fund, Arkansas EMS Foundation Scholarship Fund, or the Arkansas Ambulance Association Scholarship Fund in this section.
   
   
7. Payment Method — Check the appropriate payment method you will be using. The AEMTA will be accepting Purchase Orders for EMS Conference 2008 provided that all information is complete.
